To add, update, or edit the questions in your waiver app, share your request with [e-mail]. 

Our team can reword existing questions or add a new question for you. It helps if you're as specific as possible.

Your question response types can be:

• Yes or No
• Single Select (select 1 from a list)
• Multi Select (select any number of options from a list)
• Text Entry
• Numeric Entry
• Date Selection

We capture one response per page. If you require the response 'Yes' to ask for more information, we would add in a second page for a Text Entry, where the guest can add in additional information.
