In certain scenarios you may wish to add a page that allows a guest to sign using only their initials to agree to a certain clause or passage of terms text. You can add this type of 'initials' page to your form by following these instructions:
- Login to the dashboard, navigate to Settings/Forms and click on the form you want to add an Initials page to:
- Scroll just underneath the page after which you want the Initials page to show, and click "Insert + Page":
- The "Add Form Page" dialog appears. Switch to the Custom tab and select Page Type "Initials" from the list:
- Fill in the Page Heading (shown to the guest) and Title (for you only) boxes and click Create New Custom Property. Once it's created, click the Add Form Page button at the bottom:
- The new Initials page now appears in the form. Click "Add Question Option" and type text for the guest to initial:
Make the Initials page activity-specific (optional)
If you would like this Initials page to only appear for certain Activity, click “Add Display Condition.”
- In the popup dialog that appears, you’ll see three dropdown lists. Update them as shown in the screenshot, making sure to select the correct Activity in the final dropdown.

Final step: test your waiver
Go to the QR code tab to access your waiver link.Go through the waiver questions, and check that your new Initials page appears when you select the correct Activity.
