If you’d like to include extra questions for specific activities, you can create an Additional Form. This form allows you to ask questions that apply only to certain activities, after your Base Form has been completed.
It’s important not to repeat any questions that already exist in your Base Form. The Additional Form should only contain new questions that appear after the Base Form is shown.
Example
Base Form: General questions for all guests (Name, Date of Birth, Medical, Marketing, Email, etc.)
Additional Form (for Charter Activity): Dietary requirements, Allergies, Charter-specific Terms and Conditions
To add an Additional form: Visit the dashboard (https://dash.getwherewolf.com/)
- Select 'Settings' from the side bar menu (you will need to login as an admin user to make changes)
- Select 'Forms' from the Setting options
- Click on 'Add Additional Form'
Default Form Settings
Form Title: Enter the name you’d like to give the Additional Form.
Applicable to guest only on arrival: Enable this if you only want this form to appear on the Waiver App on the day of participation.
Default pages: These are pre-set system pages you can choose from. If you want to include questions not covered by these defaults, you can create your own custom pages once the Additional Form has been created.
When you’re ready, click Create Form to generate the new form.
Linking Your Additional Form to an Activity
Once your new form has been created, you’ll need to configure it to appear for specific activities.
Click on your new form (it will appear listed beneath your Base Form):
Under 'Additional (optional) rules of when to show this form' section - Click on Add New Rule:
In the first dropdown, select activities.
In the second dropdown, choose $in.
In the third dropdown, select the activity or activities this form should apply to.
All questions included in this Additional Form will now display for guests completing waivers for the selected activity or activities.
